When you don't know — Say so. Name what you do know, and what you need to find out.

When you see a problem — Flag it before it ships — before someone else finds it.

When you make a decision — Record why. The code is the what; the reasoning is the why.

When you estimate — Name what you know and what you don't. A number without its unknowns is a guess.

When something goes wrong — Record what happened and what you learned. A clean delivery that buries the incident is waste.
